Puerto Rican Birth Certificate Application Drive
Bring your birth certificate, photo id, marriage license and a money order in the amount of $5.00 for the birth certificate application fee. GBLN volunteers will help complete the paperwork and provide you with a stamp and envelope to mail the information to Puerto Rico.

Just in: The first 50 applicants, 59 and younger, will have the $5.00 application fee waived due to a donation by Connecticut's General Assembly Black and Puerto Rican Caucus!
